Actually, use of clothespins and clamps is pretty common in a BDSM context. Nipple clamps are pretty much de rigeur. But it doesn't need to be in a strictly kinky context. The sensation of constant pressure on erogenous zones is enjoyable to many, and this image seems to reflect that.
