Her mother is upset because she's leaving home. She's driving away as fast as she can with her doll in the back of the car and her deadbeat boyfriend in the front seat. The sign says entering "truth and consequences".

Remember your first live in relationship after leaving home? I do. It's like reality slaps you across the face. You're expected to "assume the role" and realize that you have to look amazing, cook delicious food, produce an immaculate house, contribute to the household $ and suck cock like a porn star. GirlFriday, if you are daft than I am too. I didn't get it until Carlin explained it. I will say now that I look at it more I think the mother/childhood home is the voice of truth and consequence and the young woman is driving away from that and into her fantasy life.

I clicked a few times and see that it's an illustration for "Hot Springs”,
a novel by Geoffrey Becker.  Plot: "...a volatile young woman determined to raise the biological daughter she gave up for adoption. With the help of her boyfriend, Bernice kidnaps five-year-old Emily from her adoptive parents in Colorado and ends up in Baltimore, where she attempts to make a new life for herself and the girl. Meanwhile, Tessa Harding, Emily's adoptive mother, tries to recover Emily without involving authorities.

However, Carlin's interpretation of the image is much more physiological and therefor more interesting :)
